An exciting opportunity has arisen to recruit a part time Heart Failure Nurse Specialist. The successful candidate will join an established and dynamic team providing Heart Failure care to patients in the community across coastal Sussex.

The role incorporates clinics and home visits to patients with diagnosed heart failure, supporting admission avoidance and optimisation of medical therapies. Successful candidates will demonstrate excellent communication and time management skills and will be responsible for a caseload of patients, being expected to work autonomously and able to prioritise the demands of the service. The team works closely with GPs, Cardiologists, Communities of Practice and Responsive Services to provide high quality, integrated, evidence-based care.

Previous experience of cardiology and heart failure is essential, and community experience is desirable.

A Band 6 to Band 7 development position would be considered for strong candidates.

Candidates will need to be car drivers with access to a car.
