Sussex Community NHS Foundation Trust is looking to recruit motivated and skilled Band 7 Clinical Nurse Leads who have a passion for delivering high quality care within a community setting.

Community experience is desirable but not essential as preceptorship will be offered to build your confidence, knowledge and skills within the community setting. You will be expected to have or work towards a relevant qualification at Masters level to ensure that you are clinical and academically credible. Do you have experience in managing long term conditions, and can you demonstrate advanced physical assessment skills and a problem-solving mentality towards a complex caseload? Excellent communication and leadership skills are essential as well as a desire to challenge accepted practices as needed.

We are looking for Senior Nurses to join our highly skilled Community Nursing and Therapy teams in Brighton and Hove localities. We need Nurses who are committed to delivering and supporting the health and social needs of patients within our community. The primary role and responsibility for this post is to provide complex case management to patients within a locality, working under a proactive, multidisciplinary care model, whilst remaining a part of the wider clinical team benefiting from the varied skills and experience the team has to offer. As part of an integrated service you will have access to a variety of specialist services and disciplines which are essential to our care delivery.

You will also be involved with supporting and developing our staff within the team. The clinical lead post is an exciting, autonomous role promoting and delivering excellent clinical practice within a community setting.
