Opportunities have arisen within the Responsive Services Team in the Central Area of West Sussex for highly-skilled, motivated and enthusiastic individuals to join our team. We have bases at Mid Sussex, Horsham and Crawley

As a nurse you will Join our Multidisciplinary Team which consists of an  Nurses, Occupational Therapists, Physiotherapists,  Community Reablement Assistants and Community Care Assistants. Working collaboratively with our acute Trust and other community services, including third sector organisations, we aim to improve the patient journey and pathways of care and deliver short term intervention, care and support in the community.

Responsive Services delivers the ‘right care at the right time in the right place' playing a key role in supporting patients discharged from hospital and community beds and support the patient to stay at home and prevent avoidable admission to hospital.

You will need a UK driving license and use of a car for business purposes.
