Due to upcoming maternity leave, the award winning Brighton & Hove Community Heart Failure Service is looking to recruit a Heart Failure Specialist Nurse.

Ensuring a robust and mutually well supported service, you will be joining a team who work closely with High Weald, Lewes & Havens Heart Failure Service, both are within the East locality across Sussex Community NHS Foundation Trust. For the right applicants, a job share may be considered. We aim for there to be a cross over time with you and the current HFNS who will be going on maternity leave. You are an excellent role model, sharing your advanced clinical knowledge and provide excellent patient care, able to tailor each patient's treatment plans to their individual needs ensuring that practice is holistic, evidenced based and according to Professional and National guidelines. You work autonomously, managing your own caseload and have excellent communication skills. You may be required to oversee juniors' caseload to ensure safe and effective care planning. You liaise closely with members of the multi-disciplinary integrated team including Consultants, GPs and other Health Care Professionals.

The service offers regular supervision and opportunities for continued learning and support. You will provide specialist expert nursing in Heart Failure care to patients within their own homes and community clinics. We will be able to support you with this. We are a proactive team in developing the specialist skills and are embarking on an exciting service development opportunity which your support and knowledge may help to succeed.

The team also benefits from a very close working relationship with a fast developing Secondary Care Heart Failure team. BSUH is a nationally recognised tertiary centre in Cardiology offering innovation in Heart Failure through weekly Heart Failure ward rounds, weekly MDT meetings, Ultrafiltration for diuretic resistance, day case IV iron and access to devices including the development of remote monitoring. The teams work in a collaborative way with weekly clinical supervision in the form of MDT meetings and regular joint teaching. These are a valuable support for the community team. All supported with HF Consultants, SPR's with input from Pharmacists, Physiologists, Psychologist and Radiologists.

The community heart failure team has strong links with the British Heart Foundation and the British Society of Cardiology which ensures regular updates in heart failure management, conferences and teaching as contemporary practice and service development is bedrock of the team's approach.

Essential requirements for the post are:

* First level registration on the NMC register

* Evidence of Degree/Masters level study

* Minimum two years heart failure experience

* Evidence of excellent attendance

* Flexible and dynamic approach to service delivery

* Excellent communication skills written and verbal
