Northern Prevention Assessment Team NHS Nurse Advisor - Band 5 30 Hours Per Week Location - Horsham This post forms part of an exciting multi-agency team working in Horsham, Crawley and Mid Sussex areas as part of the Prevention & Wellbeing Programme (PWP). The objective of the service is to promote person-centred and joined up approaches from Health; Social Care and Voluntary agencies which should: • Promote health; well-being & independence for adults and older people. • Prevent unplanned acute hospital admissions and delay the need for higher levels of health & Social Services and admission to residential care. This is an anticipatory; health promoting model of care for adults and older people and includes the identification of those at risk living in the community. By an holistic assessment of health; social & environmental needs and the provision of advice; signposting and assistance with accessing a range of statutory and voluntary services; this model of care has previously been proven to be successful across the County as part of the Partnerships for Older People Project (POPP). The Northern Prevention Assessment Team is well established in the locality. The PWP builds on the POPP framework by organisations working together in locality based teams to improve accessibility and referral onwards to the most appropriate agencies who will meet the needs of the individual. It builds on the findings of various papers including: National Service Framework for Older People; A Sure Start to Later Life & Healthy Lives; Healthy People. We are looking for someone who has a nursing or allied health professional background with knowledge of community working and who can demonstrate a good level of post registration experience. You will be flexible in your approach and committed to close partnership working between health; social care & the voluntary sector. You will have excellent communication and inter-personal skills and an up-to-date knowledge of issues surrounding the health and well-being needs of adults and older people.
