Job Details
St Barnabas House provides specialist palliative care to adults in the Worthing, Adur, Arun and Henfield areas through a range of services, including in-patient care, day hospice and community care.
We are looking for an experienced social worker who will provide a social work service to patients and their families, particularly in the community, working closely with other members of the Family Services Team and the hospice’s multi-professional team
You will need to be a confident and competent social worker with excellent verbal communication skills and the ability to work with adults and children:
- Undertaking psychosocial assessments of the needs and resources of patients and their family members
- Providing ongoing psychological support, advice and information to patients and their families, primarily in the community
- Working with individuals and other family groupings
- Engaging in group support for children
- Liaising with other care providers and organisations for the benefit of your clients
