Grade - NHS AfC: Band 4

Sussex Community Foundation Trust is looking to recruit dynamic, motivated and enthusiastic Band 4 Assistant Practitioners that have a passion for delivering high quality care within a community setting.

Community experience is desirable but not essential as preceptorship is offered and tailored to your needs, to build you confidence, knowledge and skills within the community setting.

To be awarded a Band 4 post you will be expected to have successfully completed the Foundation degree. We are looking for Band 4 practitioners to join our highly skilled Community Nursing and Therapy Teams in Brighton and Hove Localities. We need staff that are committed to supporting the delivery of health and social needs for patients within our community.

As part of an integrated service you will have access to a variety of specialist services and disciplines which are essential to our care delivery. You will share responsibility for a caseload of patients supported by a Case Manager, and as part of the wider team benefiting from the varied skills and experience the nursing and therapy teams have to offer. You will be required to work autonomously but will have access to support where necessary to ensure quality care delivery.
